hey chowdary.
get an application for NJ board by calling them and you have to fill out the 1st form and the preceptor form needs to fill by your pharMACIST and along with your application you need to send a notorized photocopy of FPGEC or your Score report and call the ETS to send your TOEFL and TSE directly to the board.along with your application send a covering letter saying that NJ board should request the FPGEC score report directly from NABP.
