Oh, I now see that was part of your original question. Unfortuantely, most, if not all, of the programs I mentioned in my post are not known for offering conditional admission since they are rather competitive for admissions, and I'm not familiar with which departments are known for conditional admission, though many do state on their webpage that they sometimes offer it:

It may help if you mention why you think you may you need conditional admission. Is it because of prerequisites? If so, UT Dallas and maybe some other CS programs don't require much prerequisites prior to admission for the MS in CS. Or is it because you want admission for Fall 2008 or some other reason?
