I am South African but I wrote my exam in April.
As far as study materials, I just used CPR, over and over, till it was coming out my ears, and I found the exam very manageable. Bear in mind that I qualified 8 years ago and only worked in retail for about 6 months back then.
I am applying for an H1-B in California - apparently it can take 4 months from the time you submit your documents. And in California you need to submit fingerprint cards, so request those now if you are planning to go there.

I found the NABP took quite a long time to send my certificate - over 2 calendar months since I wrote the exam. I thought it would be quicker with the new exam administrator and computerised tests, but it wasn't. And you have to have sent them your TOEFL results before they will do your certificate - the TOEFL results are on the certificate.
