I've just realized that I cannot accept the offer to attend graduate school this fall. Family reasons make it impossible. Problem is that I already accepted the offer some 2 months ago. What should I do? Is the university going to be mad? What adverse action can they take? It is really unfortunate but I can't attend.

I would think you should inform them ASAP that you will not be attending, i.e. that you had an unforeseen family issue. They may offer a deferred admission for Spring of next year if you request it.

Without any intention of terrifying the OP, doesn't this entail that the OP's application to the same program again will be perceived negatively.
