Can someone please assist me regarding my NABP application?
Hello.
Can someone please assist me? I graduated from the University of Santo Tomas in the Philippines in 2002. I am in the process of applying and sending in my $800 to the NABP. I have a letter in a sealed envelope from the Professional Regulation Commssion that reads: "Certification To whom it may concern: This is to certify that beau-geste passed the board examination from Pharmacy on such and such date and as such issued this license number: This further certifies that as a passer, he is entitled to practise his profession in the Philippines." So, is that all I need to submit, other than the $800, 2 photographs, and application form? Does this letter in a sealed envelope meet the requirements, "Official Proof of Licensure and/or Registration"? I'm sorry, but I just want to make sure, as it is confusing to me. The application number 6, A."Was an unrestricted license required to practice Pharmacy (in the Philippines)? and 6. B. "Was an unrestricted registration required to practise pharmacy (in the Philippines)? How do I answer? Yes for both? If I do that, will the NABP require some other official letter? It's just so hard to get anything from Filipino officials, especially since I am in the USA. My ECE report was completed and sent in last December 2008, but it took awhile to get the certification letter from the Philippines. Also, do you guys think that I need to make a notarised photo copy of my ID card and my board of pharmacy test results and my actual pharmacist license certificate? Quote:

The proof of licensure should originate from the philipine board of pharmacy, i.e. the authority that license practicing pharmacist in your country, they will send it directly to N.A.B.P. next time make your questions appear simple, it looks complicated and difficult to follow, forum members may not have time to read a whole lenght

Please assist me regarding my NABP application too?
1. For the Photographs How can I prove he photos have been taken within three months? Do I ask for the photo taker to put the date on the back of the photos? 2. For the additional supporting Documentation. How many supporting documents need ? The first page of the passport is enough or need driver's license as well. Thanks. |

Hi Cachan,
There is no proof needed for the photographs. They just want to make sure that seeing your photo on your ATT(entrance pass to examination hall) at the examination hall that they should identiy that you are the one who has submitted the application. Dear Netbuddy and vvfong,
Thank you for your response. I got another question about where to attach the un-notarial photo? One photograph will glue on the application form, where another photograph goes. Glue on the same place(box) or just pins it on the application forms. Thanks. |

Cachan, the other photo you just attach to the application form using a paper clip but do not staple/glue. I guess it's for the certificate once you pass.

Hi vvfong,
Thank you for your response. Sorry for one more question about the pencil. According to the application information: When marking the application form, use only a No 2 (soft lead) pencil. My question is: “ does HB/2 pencil same as the No 2 pencil ? “ THANKS. |

Thank you for your information. One more question about the money order/ bank draft. Do I need to write down my name on the draft? If so, should I write on the top of the draft or write name on the back of the draft? Thanks. |
