The statement says that they will miss two thirds , who are carrying infection and will not get screened .
The argument assumes that this two thirds does not carry any infection and so they never get screened and hence are again a source of infection .
I don't know if i am able to put my point across .....the question basically says that even after screening there would be a considerable proportion of population that contributes infection and the reason is that they never get screened not even for other infections and so are unaware of their infection status.
