Hi lipoprotein,
According to me, it should be # 4(both 2 & 3). I would give this definition for the explanation : Therapeutic interchange is the dispensing of medications by pharmacists that are chemically different, but therapeutically similar, to the medication prescribed.So, according to the definition, thought all PPIS are from the same class, and therapeutically similar, but they are chemically different. Correct me, if I am wrong.

At the pharmacy I work, it is still 100% approved or atleast notified to a prescriber, I guess this could differ in states and also for different drug categories.

Therapeutic substitution should always be asked permission for from the prescriber.Generic substitution is the only change you can automatically do when dispensing.
