Hi all
I need some help here.
I want to apply for intern permit for MIchigan and have a question for filling out the intern application.
On the last page of the application, there is a section requiring College of Pharmacy Affidavit. Do I have to send this form to college I attended and have them to complete it? Or foreign pharmacy graduates don't have to do it?
I need your help.

thanks for your info and I have one more question for the application.
I have FPGEC and I will send a xerox copy of it along with a xerox copy of my fpgee score report I received before. Do I need to have nabp to send my fpgee score directly to the board? or just xerox copies are ok?
and how long does it take an intern permit after sending all these materials?

Hi taki just send any xerox copy of any document u have to the michigan board they will verify ur fpgee score by itself from the NABP & if u passed TOEFL & TSE let ETS report ur scores directly from them the time is approximately 3 weeks....................best of luck!!

As far as i know u don't have to fill that if u are a foreign pharmcy graduate, or u can email (mostly u can expect Ans in nextday)or call state board.
