Hi,
Only certain institutions can sponsor J1 visas, and to do so they need to have an existing program approved by the Dept of State. Those institutions that have such a program for pharmacist interns are research hospitals or federal agencies (VA, NIH...). In my experience research hospitals don't take foreign interns requiring visa sponsorship. J1 visas sponsored by federal agencies usually come with a 2-yr home requirement (when the J1 expires you have to go back to your home country for 2 years - unless you obtain a waiver - before coming back to the US under any other visa type).
