AJRAYES,

You need to have at least 100 posts to be able to download a document. However, I am contacting Erin and trying to make it accessible by other ways.

I am also searching for a server to upload a html document. You can also send me email address through a private message system. I will forward the application in MS Word to you.

Click on this link MegaShare.com Free Webhosting, scroll down and choose free, then all you have to do is wait 10 seconds, and after that it's all yours, just download and complete the form.

Don't forget to enclose a copy of NABP correspondence or letter that contains your EE number, please.
