Well, I would say that if your are considering US schools that they would appreciate schooling in the US so that they know you can succeed at a US college. Your class work experience is a little old and not really preparation for a phd program. The work experience is a plus. GMAT is good. You might want to try and improve your LORs. Overall, I think the biggest obstacle you will have in admissions is the fact that most phd applicants have taken many steps to prepare for phd programs (either in research, teaching, classwork, or by scoring really high GMAT scores). Your qualifications don't necessarily show how you have been preparing for a phd in operations. That being said, I think you can apply to many schools and find success. I don't know if we have an operation guys on the forum, but I defer to them for their input on schools.

You should aim a little lowish in the rankings. This depends largely due to which Indian university you are from. If you are from IIT or IIM, then you are good in anyway. However, if you are not, you have to aim a little lowish in the rankings.

I know there are quite a few rankings for accounting. PAR report presents one, so does financial times, and there have been articles written on rankings that can be found on SSRN or even on this board. That being said, I really don't know of operation rankings.... Sorry.
