Has anyone received a conditional admission from a PhD program? Basically, the letter saids I have to maintain a certain GPA since I am in the last year of my masters. I've gotten numerous offers that were unconditional so I was surprised to see this condition. For these cases, is the offer normally rescinded if my GPA falls below the cutline? I really can't afford to take the risk of giving up my other offers if that is the case.

Probably, you have not met all of their requirements, such as GPA.

No I met the GPA requirement when I had submitted my application.
