Title?

On the recommendation forms, when they ask for the title of the recommender, what exactly are they asking? should I be putting "Dr.", "Mr." and "Mrs", or do they want things like "Associate Professor" and "Assistant Professor". I feel hopelessly niave here, but i would hate to guess and offend my recommenders. Thanks you.
