Lost TOEFL Examinee's score report -HELP!

I Had appeared for CBT Toefl in may 2002.(score:277)

 

However, I recently found out that I have lost My TOEFL Examinee Score Report

 

Fortunately,I have the TOEFL 16digit Appointment Number

 

How can I order TOEFL Examinee Score Report ?

The latest TOEFL Bulletin has a form for additional toefl score report but no form for additional TOEFL Examinee Score Report

 

 

Please Help as I need TOEFL report for my VISA Application.

I had a similar problem once. All you need to do is write them a note at toefl@ets.org, and inform that you need another 'Examiner's Score Record'. The corresponding emails I sent and received are as follows:-

Educational Testing Service

 

Subject : TOEFL CBT Examinee's Score Record Not Yet Received

 

Sir/Madam,

This is to bring to your notice that I, (My name), have yet to receive the Examinee's Score Record, even after the stipulated time period of four weeks. The details pertaining to the test are given as follows:

 

Test : TOEFL CBT

Name : (My name)

Address : (My address)

Test Date : (My test date)

16-Digit Appointment Number : (My 16 digit appointment number)

 

You are therefore requested to do the needful at the earliest.

And a week later, I received a reply:

Regarding your inquiry:

 

We are sorry to learn you did not receive your examinee score record. We are sending a duplicate score record to you.
