Mailing Score

I am now applying for uni. and I am having problem to mail/fax the uni. official score. I went to www.toefl.org
but they just showed the form there but it's not downloadable.
Can anyone give me a link or a file that I can download it and then I can fax it.

Have you received the official score report? if yes, you can make some copies and fax them to univs.
BTW, you should check whether you can send your score by fax/mail because most of the univs require the official score report sent directly by ETS. You have to contact ETS and tell them to send the report for you; that will NOT free of charge.

Yeah, I got my score in Jan,2003.
And they said I can't use the form that they gave me because it says don't use it after june or july.

And yes, I know most uni don't accecpt mailing/fax.
I wanted ETS to help me send my official score and I know I needa pay for it.
But the problem is, there are 3 ways to ask ETS to help me send my score to uni.
Telephone($12 for service charge + $15 for sending) ... it's too expensive.
Or I could send ETS a mail, or fax them the form and ask them to help me send my score.

I couldn't find anywhere in the site that I can download the form. They have the form in a page but it cannot be downloaded or right click.
So I am wondering if any of you guys can help me out by giving me a link to download the form or send it to me through e-mail,msn,icq.

Oh, I see.

Just right click on the form and print it out. Then, you can fax or mail the filled form to ETS.

Yeah, I tried but the thing is, it prints out the whole page. And it also cuts some part on the right hand side.

I can't just print the form because the form in the site is not a picture format.

HELP?????

Ohhh, I've just remembered a thing. You received the official score report, didn't you? That report includes the form you're looking for. Just cut it out and make some copies because you will need them sometimes.

Yeah, I knew it.
But when I get it out. There is a sentence that said don't use this form after june or july 2003
