Question about the admission ticket

Hi, there:
Could somebody tell me do we have to fill the institution code and address of the institution on the TSE admission ticket or we could leave it blank if we do not wish to have scores reported to the institution at this time?

Yes you could leave it blank if you want to or either if you know the code of your institution just put it in , by the way are you entering school or graduated professional ? just curious , good luck.
