Admission Ticket

Hello, I'm having a little confusion here, I got my TSE Admission ticket for the June 4, 2004 exam. The portion below is the TSE score report mailing instruction form. Is it automatic that the result will be sent to my home address? - or the address I provided in the application form. I don't have any institutions where my score reports will be forwarded to. what do I put in the 2 slots provided?

If u r taking the test 4 some licensure purpose write their code.If they dont have any,leave it blank.I wonder 4 what purpose u r taking the TSE.

I need the TSE for the FCCPT (credentialling board for Physical Therapist). I haven't filed my FCCPT application yet but I am hoping to file it not later than August. I was wondering if it's ok if I have my TSE result forwarded there since I didn't apply for FCCPT yet, and also, I cannot find the institution code in the TSE booklet, so it's 1000 for the code right and 00 for the department code?

It must be 1000.But I think that they might be having a code.Just mail the FCCPT & ask 4 their code.If they don't hvae one(which is unlikely fill it with 1000 & 00.

U sure can forward your scores 2 FCCPT even if u have not applied to them yet.taht is waht I too have done .I have nt applied 4 FPGEE but I have sent them my scores.

Thank you for all your answers... I got the code, it's all there in the Bulletin, I was looking under another state thanks again.. Wish me all the best for the TSE... will take my exam later...
