Regarding the IELTS, it should become a valid conduit for health care workers to pass the English aspect of visa screening as related by others here. With the IELTS you do not need the TSE. However, if you are taking the TOEIC you will also need to take the TSE exam. There is no spoken English aspect of the TOEIC. Please refer to US public law.

The intent was never to make things easier, just to open more avenues for examination when the MELAB was discontinued.
