DS-156 FORM [QUESTION 30]

Hi there,

I was filling out my DS 156 form for my student visa, Need your advice for question 30. I did my masters in United States and worked for couple of years with H1B visa. Here is my question, In DS-Form Ques 30

To specify the date for my Most recent visa, Which date should i specify
1. My H1B visa was approved for company A on May 15, 2007 by USCIS
2. My H1B visa was transferred to company B on may 23, 2008 by USCIS
3. I got my h1b visa was stamped in india on dec 12,2008

If option 1 or 2 is the right answer, Then my next question is
"Where my visa was issued. Should i have to specify United States of America"
