Its better to take toefl IBT than TSE. Its more easy to get desired score in toefl IBT including analysis on ur score. However the test is time consuming.

APHA complete review of pharmacy is enough www.rxlist.com for top 300 drugs and any additional info abt drugs How much time?? donno... changes from person to person I guess. My recommendation is to take the test once u score atleast 100 in PRE-NAPLEX

Yes, but they r not exactly evaluation forms. They r intern report forms. You have submit signed forms for every pharmacy you worked. Also check the board web site if there is any dead line for reporting. Some mandates monthly reporting, some quaterly and some after you complete all the required hours. Also some boards request to report even if you not working. Many boards also requests you to get a letter from ur preceptor before applying for pharmacist license. This is more like evaluation form and you can get this from any pharmacist you worked (usually from pharmacist u worked more hours) and have good relation.

I dont think you wil have any problem visiting US as you did it 2 times in the past and returned to your country. Its best to tell them the exact reason for your visit. Coz, visiting USA to write an exam is not an uncommon or suspicious thing. TOEFL, NABP etc dont release your info to others without your permission. So dont worry about them.

You can easily transfer intern hours from state to state just by writing letter to the state board where you got your hours. Yes, you can work as many pharmacies as you want as long as the hours are properly signed by your pharmacist incharge. Also look for state board of pharmacy websites for exact procedures.

You can apply for internship either in Florida or Michigan. Email them to send application for intern.
