It completely depends on the money you are willing to spend. My suggestion would be that you first apply to a wider range of programs, where you can include programs like Duke's or MEDS or b-schools, along with more common programs to where you think your profile is worth giving it a shot. And later, only after you are accepted, take the decision on where to go. Although you sound pretty convinced on what you want to do, it could be the case (I don't know you, nor your profile's strength) that you are rejected in all specialized programs, which would leave you without any options in the first place.
