Hi Friends, I have a question regarding the perceptor evaluation forms that we submit after completing internship.Do we have to specify in the forms that....in some weeks we put 45 hrs/wk & in some 40 hrs/wk. Hello, The required documents are:- 1]The NJ board needs official FPGEE report. By official reports they meant the original FPGEE score. So send a letter along with the application saying that " the NJ board can request the score report from Nabp". 2] Call ETS & ask them to send your Toefl/TSE scores to them. I think it is 9359. You can look it up in ets.org site.Or if you are planning to call them you can confirm it with them. I'll also try to post the exact code tonight as soon as I'm home. The NJ Board takes at least 2-3 weeks to process the application. All the best

Hi dr_tamer81, I sent the documents last week & haven't heard anything from them yet. I have sent the following documents:- 1) Application for intern 2) Application for Preceptor 3) Notorizsed copy of FPGEE score report. 4) A letter saying that they can request my official score report from Nabp. I asked ETS to send my Toefl & TSE scores directly to NJ board. I'll keep you informed when I recieve my intern permit.:)
