Nope. A credit card/bank card with a sig can be a secondary.

I know we need to bring an I.D. with photo and signature. What else can we bring? Are they going to ask us for the ATT letter, as well? If we don't have the proper credentials, they're not going to reschedule the test so we have to schedule and pay again. So, I'm hoping this won't happen and we can be ready when we get there to our testing sites. Hi Pallmall. I had the same question before only because I can't seem to find a hospital where I can do my intership at. I emailed the Board and asked about this and they said not to give up because we are required to do both community and institution to complete the 1500 hours. It does not matter how many hours you do for either as long as you do both. So, basically, they have no required minimum hours on each, that was what the correspondence stated. I still think though that the Board should have a clear cut information on this at their website because it leads to so much confusion.

Congratulations! You're just a small step away! =) Right now, I'm still collecting materials to review although I have Kaplan and APHA. I wish I have notes where I can study from, but I have graduated a looong time ago already so I'm kind of relying on those books as my main resources. Hello! Does anyone know what are all the requirements needed to be submitted after finishing the internship hours and the address to submit it to? Do we need time sheet as a proof of internship hours or just the form from the ca.gov of your completed hours? DO we need to submit anything else other than the internship form?
